VA Power of Attorney for Health Care:
1. What: "V.A. Power of Attorney" for health care.
2. How: You can go online at VA.org and get VA-Form 10-0137 or print this attached form.
3. Why: You need to get this Power of Attorney now. If your spouse is not able to write or talk and you have to make decisions, you will need this "Power of Attorney" to make this decision for his/her medical care.
4. Fill out these forms and take it to the Huntsville VA Outpatient Clinic: 500 Markaview Road, Huntsville, AL 35805. Tel: 256-533-8477.
5. Also it will be a good idea to talk to a Lawyer and have a "Living Will" drafted, and ensure that it also inludes a "General Power of Attorney".
